The Mission of Community Health is to serve as a resource in coordinating health care, environmental, and social services by providing quality health care through various programs to the Cherokee communities; in a way that is individual to their needs and consistent with their culture.
Community Health is comprised of 14 staff. Six Registered Nurses (RN) and 7 Community Health Representatives (CHR) serve the Qualla Boundary, Snowbird and Cherokee County as the need arises. RN’s provide skilled in-home and community visits for case management and medication management, with an emphasis on disease prevention and health promotion. Individualized services are available to community members who are eligible for services at Cherokee Indian Hospital. Community Health also has an administrative assistant who helps handle numerous patient requests and coordinate resources by phone for those in need.
Nursing Services offered to the public include:
- Job site and community health screenings
- Coordination of and participation in health fairs
- Communicable disease management (tracking and surveillance)
- Home visits
- Patient Education
- Liaison between doctors and patients
- Assessments and referrals for medical services
- Occupational Health services to various worksites for education and immunizations
- Vaccination clinics for communicable diseases
CHR Services include:
- Home visits in the community for assistance with medical and social needs
- Home Safety checks
- Explaining available resources/services
- Assisting with necessary paperwork to receive services
- Coordinate medical and social services appointments and transportation
- Medication/supply pickup and delivery to patient homes or clinics when no other resource is available
- Coordinate and assist with community based health fairs and prevention activities/ presentations
- Cherokee language interpreter available
